Procedure to Obtain your Driving Record

Appeal of Individual Requests:-

You should request for the copy of your own driving record in person at any branch office of the DMV or by mail by providing the following:

The Completed Driving Record Request (DMV-101-PS1) form

Acouple of copies of your state issued photo identification

A $5.00 processing fee and also you may provide your name and driver’s license number or

A $6.00 processing fee if you do not have any of your driver’s license number (It must include DOB and/or SS Number)

Let you clearly indicate where the driving record is to be mailed. If it is applicable.

All the Other Driving Record Requests:-

The Completed Driving Record Request (DMV-101-PS1) form

The Completed Release Authorization (DMV-101-PS2) form

A Xerox copy of the requestor’s state issued photo identification

A fee of $5.00 processing for the request if you have the driver’s license number and name or

afee of $6.00 processing fee per request if you don’t have the driver’s license number. (It must include DOB and/or Social Security number)

All requests for a driving record on an employee or a client must have a completed Release Authorization (DMV-101-PS2) form and must be mailed to the below address.

You may not obtain the information about others without their written consent on the Release Authorization (DMV-101-PS2) form or unless the request is made on the company or business letterhead and provides a legitimate and the detailed reason for the request as defined as the Uniform Motor Vehicles Records Disclosure Act.

Alternate procedure to Get Your Virginia Driving Record via online

You can obtain your Virginia driving record through any of the following three options listed below:

1. Online - visit http://www.dmv.Virginia.gov/dmvnet/common/pin_option_ques.asp. (you will be charged with a fee of $8)

2. Visit any customer service center present of your identification card and $8 fee

3.By the source of mail send a written request including your full name and address and social security number the reason you want your request and $8 to the following address.
